Instructions

1. Review availability on the Calender page. Any day that is higlighted in RED is Not Available. The Calender can be viewed by selecting the "Calender" option in the menu at the top of this screen.  

2. View and Print the form by a Left Click or Right Click here : Form View and Print Using a Right Click, select "Save As" on in the pop-up screen to save the file on your local hard drive.

3. Read all instructions on the first page carefully.

4. Read all Rules on the second page.

5. Contact our office for the following 

   • Payment Instructions

   • Form Mailing/Handling/Faxing instructions

   • Any questions you might have

   • Affiliated Congregations please contact our office for discounted rates.
